CYGNVS Launches Platform to Manage AI Failure Crises
CYGNVS has launched an AI Incident Command Center to help organizations manage crises caused by their own AI deployments. The platform addresses failures ranging from biased outputs and hallucinations to rogue autonomous agents, using an isolated architecture that keeps response systems out of reach of the AI being investigated.
The launch comes as AI incidents surge, with the OECD recording 596 AI incidents in January 2026 alone, a 200% year-over-year increase. The platform covers preparation, practice, response and reporting, with templates spanning 56 binding laws and 47 frameworks globally, and is generally available now.
